APPOINTMENT INSTRUCTIONS
Follow Up as scheduled
- Send an Email for an earlier appointment to discuss medication problems or changes
- Call your pharmacy when you need refills - please allow 5 business days
- Call your insurance company for questions about co-pays or insurance coverage
- Medications refills are provided until your next scheduled appointment
- If you have missed an appointment and are out of medications:
Send an Email for an appointment to discuss restarting medications
GENERAL INSTRUCTIONS
- Do not use drugs and alcohol
- Do not drive if sedated or impaired
- Weapons need to be removed or locked
- Complete Labs as directed
- Take medications as prescribed
- Remove any unused medications
- All medications need to be locked and managed as prescribed (Video Link)
- Do not become pregnant on these medications (Video Link)
- If you do become pregnant, notify your providers in this office, your PCP and OB-GYN immediately
- RETURN TO THE CLINIC FOR ANY INCREASE IN SYMPTOMS OR SIDE EFFECTS
